Event Description:
  Join Dr. Steven Pantilat as he shares about the importance of Palliative Care and how to Live Well with a Serious Illness.
Dr. Steven Pantilat is a palliative care physician and a hospitalist, and is an internationally recognized expert in both hospital medicine and Palliative Care. Dr. Pantilat is the Director of the UCSF Palliative Care Leadership Center that has trained teams from over 200 hospitals across the country on how to establish Palliative Care Services. He is the Director of the Palliative Care Quality Network, a national collaboration of over 85 Palliative Care teams focused on improving the quality of care for seriously ill people. Dr. Pantilat is the author of Life After the Diagnosis, Expert Advice on Living Well with Serious Illness for Patients and Caregivers. Dr. Pantilat will be available for a book signing at the end of the event. A small quantity of books will be available for purchase at the event.

Event Description
This event is designed to educate community residents and healthcare professionals on end-of-life issues. Continuing education units are available for RN/LVN/CNA/HHA/SW. Lecture will be the primary teaching technique.

Objectives:
• Describe three cultural myths that are barriers to good care for people with serious illness like Alzheimer’s Disease and how to overcome them

• Describe communication tools to help understand patient goals and values

• Describe how palliative care can help people with serious illness like Alzheimer’s Disease live well and long

Continuing Education Details

RN/LVN/CNA/HHA: Provider approved by the California Board of Registered Nursing; Provider No. CEP 6818 and NAC 1033, for two (2) contact hours. SW: Two (2) CEUs and a Certificate of Completion will be issued by California State University Stanislaus, Office of Extended and International Education.

Registration Information/Cost

Event size is limited to the first 140 participants. Please register early. The cost for this event is $45 per participant. Event fees include dinner and CEU units. Payment in full is due when registration form is submitted. Confirmation will be emailed if an email address is provided. If written cancellation is received 10 days prior to the event, a full refund will be returned within 30 days. No refund will be given if cancellation occurs less than 10 days prior to the event.
